Save or download this Microsoft Access file, ETRANS-CodingMatrix.mde to the folder located on your computer where the old file is saved. In most cases a folder named ETRANSv2b in your C Drive is where this old file was downloaded. Some companies use a shared drive for this folder.

Save the file to the folder and overwrite the old file.

L/A&H Universal Transmittal Application

Effective January 1, 2006 insurers will have two different ways to make policy form filings. The new requirements are outlined in 50 Illinois Administrative Code 916 which is available on the Illinois Department Insurance website.

The option to use the System for Electronic Rates and Forms Filing (SERFF) will continue but beginning January 1, 2006 there will be an alternative to using SERFF. Insurers will be permitted to place their entire policy form filing on a CD ROM by simply following the instructions on the Department's website, insurance.illinois.gov, clicking on Insurance and drilling to Industry and then Regulatory Filings and then clicking on the appropriate bullet in the Table of Contents. The prepared CD ROM is mailed to William R. McAndrew, Assistant Deputy Director.

The Universal transmittal form (Life, Accident and Health, Annuity, Credit Transmittal Document) will replace the Product Evaluation Database System (PEDS) General transmittal form, Informational transmittal form, Replacement/Withdrawal transmittal form and Certificate of Assumption transmittal form currently used in the policy form filing process. This transmittal is used for filings for Health Maintenance Organizations, Limited Health Service Organizations, Voluntary Health Service Plans, and traditional Life, Accident and Health form filings. Since the Universal transmittal form contains an attestation section (to be signed by a company officer) that the forms contained in any particular filing comply with our statutes and rules, the Annual certificate of Compliance filing will no longer be required. Each Universal Transmittal must have a unique Company Tracking Number. Please ensure that the first 14 characters or less of the Company Tracking Number uniquely identifies the Company Tracking Number from any other tracking number previously used by the company. The Company Tracking Number cannot contain symbols other than hyphens or parenthesis. Do not use spaces or underscores. These characters may prevent your filing from being processed.

Illinois Medical Professional Liability Insurance Uniform Claims Report

In 2005 the Illinois General Assembly passed, and Governor Blagojevich signed into law, PA 94-0677 (the Act) that addresses certain aspects of medical malpractice business written in Illinois. The Act effectively changed the reporting requirements of Section 155.19 of the Insurance Code (215 ILCS 5/155.19). As a result of this statutory change, the Department of Insurance amended our administrative regulation (50 Ill. Adm. Code 928), which sets forth the required form and procedure for filing Illinois Medical Professional Liability Insurance Uniform Claims Reports.

Beginning January 1, 2007, any insurer or any other entity authorized or permitted by law to provide medical liability insurance in this State shall report to the Department of Insurance all Illinois medical professional liability claims and suits pursuant to Part 928, which requires electronic reporting utilizing the filing software provided. Insurance Data Reporting Requirements

  1. Part 4203 Subpart A Insurance Data Reporting Requirements:

    Subpart A is due November 30th of every year. The format, specific data elements, and filing process required for the November 2011 data filing are substantially identical to the November 2010 data submission process.

    The Department asks that all data be submitted through the online application. The online application will verify the format of all data submitted, replacing the edit program previously utilized during the Part 4203 data call process. Companies will no longer need to download and install the separate edit program.

    In the event you do not have Access 2007 available, you may use the excel file provided here. The completed spreadsheet may be returned to the DOI using the same process as described in the access application procedures. Please note that the excel template requires each company to be submitted separately. The access application allows for multiple companies in a single filing.

Please be advised that the Department of Insurance has suspended the collection of the Agent Zip Code Data Call and the Administrative Zip Code Data Call at this time.  All companies required to file the data contained in either of these data calls will be notified if and when they are reinstated.
